Jan Fatylak, 65, of Blakeslee, passed away on Monday, August 7 in his home.

He was the husband of Aleksandra (Czetyrko) Fatylak. They celebrated their 44th wedding anniversary in April.

Born in Poland, he was the son of Justyna Fatylak.

Jan was a self-employed truck driver for many years.

In addition to his wife, he is survived by a daughter, Sylvia Damiecki and husband Boguslaw Wojtanowicz of Blakeslee; a sister, Helena Zielinski and husband Andrzej of Florida; and two grandchildren, Ashley Fatylak and Nicole Fatylak.

A Memorial Mass will be held on a later date at the convenience of the family. Cremation will take place in H.G. Smith Crematory, Stroudsburg.
